Parties or their lawyers must provide our program with a copy of the court order which specifies the terms and conditions of access (times, dates, and frequency of the visits). Both parties are required to contact our program in order to book his or her
own intake interview.
What do custodial parties need to bring when attending the intake interview?
As the custodial party (the parent or guardian with custody of the children) you are required to bring the following to your intake interview:

Your full address

Names and telephone numbers of two of your friends or relatives so we can contact them in case of an emergency

Your child’s picture

Your child’s physician’s or pediatrician’s name and phone number

Your child’s health card number

All assessments and affidavits with regards to custody and access

$25 non-refundable application processing fee (deductible from
the yearly user fee if you are accepted into the program). There is also a yearly user fee. This should be paid after your acceptance to the program but must be paid prior to the first visit. For details, see “User Fees.”

Who conducts the intake interview?
The Program Director, Intake Coordinator, or the Site Supervisor will conduct the intake interview.
