Are there any fees required for requesting observation reports?

Yes, all parties are to be aware that when requesting observation reports, there is a $50.00 administration fee plus $5.00 for the cost of preparing each observation report. Fees must be paid prior to obtaining copies of the observation reports from our program.
Urgent requests (less than two (2) weeks notice) will be accommodated at the earliest convenience. Arrangements for an urgent request must be made to the appropriate supervisor. Please note that for urgent requests an extra $20.00 will be added to the overall administration fees.
If a second request is made in less than three (3) months from the previous request, the administration fee will be reduced to $25.00.
